Heavy debris follows two rules and both are absolute
First, it goes in a small bin. Inert material weighs roughly a tonne a cubic yard, so a 20 or 40 yard filled with it would be too heavy to lift onto the truck. Only the 6, 10 and 14 yard bins can be hauled full of it.
Second, it goes in on its own. Clean fill is taken to a recycling or inert facility where it is crushed and reused, and that facility will refuse a load with general rubbish through it. One bag of household waste redirects the entire bin to landfill at the general rate.
Between them those two rules explain nearly every heavy-material surprise on an invoice. Loaded correctly it is one of the cheaper things to dispose of; loaded into the wrong bin or mixed, it is one of the more expensive. Concrete and clean fill covers the job side, and weight limits the arithmetic.

Four steps for a heavy debris load
Say what it is
Concrete, brick, asphalt or soil. Each has a slightly different destination.
Take the small bin
A 6, 10 or 14 yard. Nothing larger can be lifted once it is full.
Load it clean
No timber, bags, plastic or packaging. Not one item.
Break it down
Pieces a machine can handle. Slabs left whole are hard to tip and to crush.

Rebar in concrete is the commonest question. A little protruding is usually fine; a slab laced with heavy mesh may have to go as general waste, so it is worth mentioning at booking rather than at collection.
Treated or painted timber is not natural wood, whatever it looks like, and belongs in the general bin. Wet concrete and washout slurry are refused outright, and set concrete has to be broken into manageable pieces.
Old tile with the mortar bed attached sits between the two. In small quantities it goes in a renovation bin; a whole floor of it belongs in a fill bin. If a load is genuinely uncertain, a photo before delivery settles it faster than a redirected bin does. What we take has the accepted list.
